Patient privacy is a top concern at Encore Surgery Center. Patients are allowed to know what medical information can be disclosed, how it is presented, the steps to get copies of their records, and how these processes lead to better quality care.

  • A patient’s confidential healthcare records can be released to healthcare professionals within Encore Surgery Center to increase quality of care.
  • A patient’s confidential healthcare records can be released to insurance providers to process payment. However, patients can restrict this disclosure for any procedure which was paid “out-of-pocket.”
  • A patient’s confidential healthcare records can be released to law enforcement agencies if there’s an investigation (i.e. domestic violence, abuse).
  • A patient’s confidential healthcare records can be released to healthcare professionals in case of emergency needs.
  • A patient’s confidential healthcare records can be released to public health organizations/federal organizations in the event of a communicable disease, faulty medical device, or “biological product event.”
  • Confidential healthcare records can only be released if the patient provides written authorization. This authorization can be rescinded at any time.
  • Encore Surgery Center might contact patients regarding appointments, treatment options, and health services that might affect you. Patients can opt out of these notifications.
  • Although patients can restrict the sharing of their confidential healthcare information, that restriction might be refused if it impedes the ability to provide quality healthcare during an emergency.
  • Patients have the right to access all information, including confidential, regarding their health.
  • Patients have the right to request copies of their health records.
  • Patients have the right to amend their healthcare data.
  • Patients have the right to know who’s seen their confidential health data and why.
  • Patients have the right to make or get a copy of the Privacy Notice, via hard or soft copy, upon request.
  • Encore Surgery Center is legally required to safeguard patient privacy. All healthcare information of patients will be kept confidential. Patients will have access to knowing how this information is kept confidential. Patients have the right to be notified of a breach of privacy.
  • Encore Surgery Center will follow all terms of the Privacy Notice. Encore Surgery Center has the right to change the Privacy Notice while sustaining confidentiality of patient data.
  • Patients have the right to notify and complain to Encore Surgery Center and/or the U.S. Department of Health and Human Services in case of any suspicion of a violation of your privacy rights.
