Encore Surgery Center is currently credentialing the following healthcare providers:

• MDs
• DOs
• DPMs (podiatrists)
• Advanced Practice Nurses (NPs, CNSs, CNMs, CRNAs)

1. Complete the Provider Enrollment Form: Complete the fields below thoroughly and accurately.
2. Attach Required Documents: Include copies of your:
   • Medical license(s)
   • Board certification(s)
   • Curriculum vitae (CV)
   • Professional liability insurance information
   • Network hospital admitting privileges (if applicable)
